Barthelmess failed to maintain the stardom of his silent film days and gradually left entertainment. He enlisted in the [[United States Navy Reserve]] during [[World War II]], and served as a lieutenant commander. Pada 1927, Barthelmess bertunangan dengan Katherine Young Wilson, seorang aktris Broadway.[7][8] Namun, pertunangan tersebut dihentikan, kemungkinan karena hubungannya pada waktu itu dengan jurnalis Adela Rogers St. Johns.[9]

Pada 1928 Barthelmess menikah dengan Jessica Stewart Sargent (1900-1965). Ia kemudian mengangkat putranya Stewart dari pernikahan sebelumnya.
