Komponen komplemen 3 (bahasa Inggris: complement component 3, C3) adalah salah satu glikoprotein dengan panjang 1663 asam amino yang berperan di dalam sistem komplemen[1] yang terkodikasi pada kromosom 19 pada gen C3.[2][3]

Aktivasi C3 diperlukan untuk memicu lintasan komplemen klasik dan lintasan komplemen alternatif. Kekurangan kadar C3 akan menurunkan sistem kekebalan terhadap infeksi bakterial.[4][5]

C3 berinteraksi dengan faktor H,[6][7] dan faktor B yang telah teraktivasi oleh faktor D berupa Bb, menjadi C3Bb, yang disebut C3 konvertase AP.

Salah satu bentuk C3 konvertase yang disebut C4b2a, yang berbentuk heterodimer dari aktivasi C4 dan C2, merupakan enzim yang mengiris C3 menjadi C3a dan C3b.

C3a merupakan anafilatoksin yang menginduksi proses peradangan, kontraksi otot, meningkatkan permeabilitas vaskular dan stimulan mastosit dan basofil untuk mensekresi histamin,[8] sedangkan C3b berfungsi sebagai opsonin dan aktivator lintasan kekebalan alternatif, juga sebagai pengikat C5.[9]

C3b menunjukkan tioester reaktif yang akan mengikat semua jenis permukaan yang memiliki nucelohile seperti amina primer maupun gugus hidroksil. Faktor I lebih lanjut mengiris C3b menjadi C3c dan C3d yang berguna untuk menunjang respon kekebalan oleh sel B.

Di dalam lintasan komplemen alternatif, C3 teriris oleh bentuk C3 konvertase yang lain, yaitu C3bBb.

Rasio C3 dalam plasma darah sekitar 75-135 mg/dl,[10] rasio yang berlebihan dapat merupakan indikasi adanya:[11]

sedangkan kekurangan C3 dapat berakibat klinis:

  • Infeksi bakteri (khususnya Neisseria), Cirrhosis, Glomerulonephritis, Hepatitis, Hereditary angioedema, Kidney transplant rejection, Lupus nephritis, Malnutrition, Systemic lupus, erythematosus
